If you are new to the area, or just new to Stanley Dentistry, then we’ll need a little information about you, and your dental and medical history. The forms, available below, may be downloaded and printed so that you may bring the completed information to the office with you, so that you are ready to meet our staff immediately upon your arrival, rather than have to sit and fill them out in the office. As is all of the information that you share with us, the information on these forms will be stored in your record and is completely secure and confidential.
